Information we collect

Depending on the form you use, we may collect contact details, general donation history, transplant-center information, program interests, a hardship description, community preferences, and supporter or contact messages.

To protect these forms from automated abuse, our security service may process technical signals such as an IP address and browser or device characteristics. Life4Donors stores only a one-way technical identifier for its own submission limits, rather than the raw IP address.

We intentionally do not ask for Social Security numbers, account numbers, medical records, or payment-card details through these forms.

How we use information

Information is used to maintain donor profiles, connect future service forms to an existing registration, begin donor verification, review hardship requests, shape community programs, respond to messages, and send updates you request.

How information may be shared

Information may be accessed by authorized Life4Donors personnel and service providers supporting website hosting, data storage, verification, communications, or program administration. It is not intended to be sold. Additional disclosure may occur when required by law or needed to protect people, rights, or the organization.

Your choices

You may ask to review, correct, or delete information you submitted, subject to legal and program recordkeeping needs. You may also withdraw from optional communications.

Data care and limits

Life4Donors uses a hosted database, a restricted staff response center, bot protection, and submission limits. Administrative status changes are logged for accountability. No internet transmission or storage system can be guaranteed completely secure. Please avoid submitting highly sensitive records until staff provides an approved method.

This launch notice should be reviewed and updated as organizational policies, vendors, payment processing, and donor services are formalized.
